WP htaccess Control
This plugin initially raised from the issue of having my custom rules wiped from the WordPress .htaccess file on each regeneration (even though the # BEGIN and # END WordPress tags were respected).
On a second moment the functionality of my old and disgraceful Custom Author Permalink plugin was improved and merged into WP htaccess Control, with improved performance.
Having much evolved since then, WP htaccess Control provides now many features which, despite the plugin’s name, have no impact on the .htaccess file generated by WordPress but rather on the internal rewrite structure.

Features
- Customization of the htaccess file generated by WordPress;
- Removal of the category base from permalinks;
- Removal of the author base from permalinks;
- Customization of the Author Permalink Base (ie: you may translate the “author” word on permalinks to your own language);
- Customization of the Paginated Permalinks (ie: you may translate the “page” word on permalinks to your own language);
- Customization (and canonization) of the Search Permalink Base;
- Category, Author and Tag based archives (ex: http://your-site.com/category/stories/2010/12, http://your-site.com/author/admin/2010/12/page/2 and http://your-site.com/tag/wordpress/2010/12), this will also work if you’ve removed the category base;
- Maintenance mode;
- Many useful and easy to use htaccess suggestions.

I didn’t check the remove tag base checkbox, but it automatically removes it. My tag archives return 404. I’m back to version 2.7. Hope you fix these bugs soon. Thanks you.

Which version of the plugin were you facing that issue on?
After 2.7.2.1, stable versions start at 3.2+ so make sure you’ve tried that last one.
Nice plugin, makes it easy to work with htaccess for beginners, I implemented a lot of custom redirects using the htaccess file using online resources, but the problem I face is unable to test if the custom codes are working as required.
Just a thought, can the custom rules be tested from within the plugin like Rewrite Rules Inspector Plugin.A feature you could add in the next version of the plugin.
